Who it's for
- You are 60 or older, or the spouse of a person receiving the service, or a person with a disability living with them.
- You live in Nassau County, have trouble moving around, cannot cook or shop safely, and have no local support system.
What it offers
- Home-delivered meals under the state Wellness in Nutrition Program
- Nutrition counseling and education
- A check on the person when meals are delivered
An in-home assessment by a case manager is done first.
